Schedule of Application

The latest application period for the Occupational Therapist Licensure Examination (OTLE) was from September 04, 2026, to November 04, 2026.

  • OTLE Opening of Application: September 04, 2026
  • OTLE Deadline of Application: November 04, 2026

Following the important schedule are the exam date and the target release of results:

  • OTLE Examination Date: December 04, 2026
  • OTLE Target Release of Result: December 07, 2026

Requirement for Filing the Application

Kindly take note of the complete list of requirements outlined below, which must be submitted in order to successfully complete the application process.

  1. Application Form;
  2. Original and photocopy of Certificate of Live Birth issued by the Philippine Statistics Authority (PSA);
  3. Original and photocopy of the PSA-issued Marriage Contract/Certificate (for female married applicants);
  4. Original and photocopy of Transcript of Records (with scanned picture, indicating the Special Order (S.O.) number and remarks: “For Board Examination Purposes”); or in case the school is exempted from the issuance of an O., a Certificate of Authentication and Validation (CAV) from the CHED;
  5. Certified true copy of certificate of practicum training, practice placement, or work experience, specifying the institution where the placement training was undertaken, and the activities or occupational therapy services, and number of hours rendered, under the auspices or supervision of a licensed Occupational This certification must be signed and issued by the same authority or superior, and duly notarized;
  6. Two (2) colored passport size pictures with white background and complete name tag; and
  7. Other documents that may be required by the Board.

* The submission of the Birth and Marriage Certificates shall be subject to the provisions of RA No. 11909 or the “Permanent Validity of the Certificates of Live Birth, Death and Marriage Certificates”.

Foreign nationals applying for the Occupational Therapists Licensure Examination must submit the following documents and pay the appropriate fees:

  1. Application Form;
  2. Authenticated copy of the national law or international treaty, agreement or covenant to prove reciprocity for the practice of Occupational Therapy between the Philippines and his/her country as provided under Section 33 hereof;
  3. Birth Certificate;
  4. Marriage Contract/Certificate (for married female applicants);
  5. Copy of passport data page or any equivalent document to establish citizenship;
  6. Transcript of Records, with scanned picture;
  7. Certified true copy of certificate of practicum training, practice placement, or work experience, specifying the institution where the placement training was undertaken, and the activities or occupational therapy services, and number of hours rendered, under the auspices or supervision of a licensed Occupational Therapist. This certification must be signed and issued by the same authority or superior;
  8. Other documents that may be required by the Board.

For degrees taken abroad, the applicant has to submit also a Certificate of Equivalency from the CHED.

How to Apply for the Occupational Therapist Board Exam

So, you’re finally ready to become a licensed Occupational Therapist?

That’s great! Here’s a simple step-by-step guide to help you apply for the Occupational Therapist Licensure Exam.

Step 1: Make Sure You’re Ready to Apply

Before you begin your application, check if you meet the basic qualifications:

  • ✅ You’re a Filipino citizen
  • ✅ You’ve finished a Bachelor’s Degree in Occupational Therapy
  • ✅ You’ve gathered all the required documents (see Step 2!)
  • ✅ The OTLE application schedule is open (check the schedule application above!)

If you answered YES to all of these, then you’re good to go!

Step 2: Prepare These Documents and Requirements

Get your documents ready! You’ll need digital copies (for online upload) and printed ones (for PRC submission):

For All Applicants:

  1. PSA Birth Certificate
  2. Transcript of Records (TOR) with your photo and the remark: “For Board Examination Purposes”
  3. PSA Marriage Certificate (only for married female applicants)
  4. Valid NBI Clearance (First-timers and Repeaters)
  5. Proof of Payment: ₱900 exam fee

Make sure your payment method (GCash, Maya, LandBank, etc.) has enough balance. If paying online, an additional ₱20 service fee may apply.

Step 3: Create or Log In to Your PRC LERIS Account

Go to the PRC online portal:

  • If you’re new: Click “Register” to create an account
  • Already have one? Just log in

Step 4: Apply for the OTLE Online

Once logged in:

  1. Click on “Examination”
  2. Select “Occupational Therapist” as your exam
  3. Choose your preferred exam date and testing center
  4. Upload your scanned documents
  5. Set your appointment schedule
  6. Pay the ₱900 application fee (via GCash, Maya, LandBank, or over-the-counter)

Step 5: Go to the PRC Office for Final Verification

On your scheduled appointment day, bring the following:

  • Printed Application Form
  • Original and photocopies of all required documents
  • A valid government-issued ID
  • ₱21 for documentary stamp
  • ₱30 for metered mailing envelope
