Schedule of Application

The latest application period for the Optometrist Licensure Examination (OLE) was from June 30, 2026, to August 31, 2026.

  • OLE Opening of Application: June 30, 2026
  • OLE Deadline of Application: August 31, 2026

Following the important schedule are the exam date and the target release of results:

  • OLE Examination Date: October 05-09 & 12, 2026
  • OLE Target Release of Result: October 15, 2026

Knowing these dates is necessary to make sure you don’t apply too early or too late, help you prepare for the exam, and let you know when to expect your results.

Requirement for Filing the Application

Below are the official requirements you need to prepare for your optometrist board exam application.

First-Timer:

  1. One copy each of the original and photocopy of the Certificate of Live Birth (subject to the provisions of RA No. 11909);
  2. One copy each of the original and a photocopy of the Marriage Certificate (for married females, unless a maiden name is preferred to be used);
  3. One (1) original copy of the Transcript of Records with a scanned picture and remarks “for Board Examination Purposes”; and
  4. One (1) original copy of the Certificate of Internship Completion with the total number of 1,026 hours for the duration of the internship program prepared and signed by the Internship Coordinator, attested by the College/School/Department Dean/Program Chair and University Registrar with the school seal.  

Repeater:

  1. One (1) original copy of the Certificate of Completion of Refresher Course from a duly accredited institution after three (3) failures on the examination.

How to Apply for the Optometrist Board Exam

Ready to take the Optometrist Licensure Examination (OLE)?

That’s great! Here’s a simple, easy-to-follow step-by-step guide to help you complete your application properly and with less hassle.

Step 1: Make Sure You’re Qualified

Before starting, check if you’re eligible to apply:

Step 2: Prepare These Documents and Requirements

Make sure you gather digital copies (for upload) and printed copies (for submission at the PRC office).

Here’s what you need:

  1. PSA Birth Certificate
  2. Transcript of Records (TOR) with a scanned picture and the remark: “For Board Examination Purposes”
  3. Marriage Certificate (for married female applicants)
  4. Valid NBI Clearance (for both first-time takers and repeaters)
  5. Application Fee: ₱900

Make sure your e-wallet (GCash, Maya, etc.) or bank account has enough balance to cover the fees.

Step 3: Create or Log In to Your PRC LERIS Account

Go to the official PRC online portal: https://online.prc.gov.ph

  • If you’re new, sign up for an account.
  • Already have one? Just log in.

Step 4: Apply for the OLE Online

Once you’re logged in:

  1. Click “Examination”
  2. Choose “Optometrist” as your exam
  3. Select your preferred exam date and PRC testing center
  4. Upload your scanned documents
  5. Set your appointment schedule
  6. Pay the ₱900 application fee through GCash, Maya, LandBank, or over-the-counter

📌 Tips: After payment, download and print your application form. You’ll need it at your PRC appointment.

Step 5: Go to the PRC Office for Final Verification

On your scheduled appointment day, bring:

  • Your printed Application Form
  • Original and photocopies of all your documents
  • A valid government-issued ID
  • ₱21 for the documentary stamp
  • ₱30 for the metered mailing envelope

At the PRC office, they’ll check your documents. If everything is in order, you’ll get:

  • Notice of Admission (NOA)
  • Program of Examination
  • Mailing envelope

Everything is in order! Your OLE application is now complete.

Before exam day, check the PRC website 2–3 days in advance for your room assignment and other important exam reminders.
