Requirement for Filing the Application

To apply for the Radiologic Technologist Licensure Examination (RTLE), make sure you have the following requirements.

BS Radiologic Technology:

1. PSA Birth Certificate (subject to the provisions of RA No. 11909 on birth certificates issued by the NSO and local civil registries) – Original and Photocopy
2. PSA Marriage Contract (for married female applicants unless maiden name is preferred to be used) – Original and Photocopy
3. TOR with CHED Special Order Number (except for Autonomous and Deregulated HEIs, State Colleges and Universities, PAASCU/PACUCUA Accredited Programs) and scanned picture and Remarks “For Board Examination Purposes” – (First Timers and Repeaters) Original and Photocopy
4. One (1) Certificate of Good Moral Character from any of the following:
   • Barangay
   • Church
   • School
   • Employer
5. Examination Fee: P900.00 – complete
   • P450.00 – removal / conditioned

Removal Examination shall be taken only three times (3x). If the applicant fails for the third (3x) time, he/she be required to take the entire examination.

X-Ray Technology
Associate In Radiologic Technology (Art):

1. PSA Birth Certificate (subject to the provisions of RA No. 11909 on birth certificates issued by the NSO and local civil registries) – Original and Photocopy
2. PSA Marriage Contract (for married female applicants unless maiden name is preferred to be used) – Original and Photocopy
3. TOR with CHED Special Order Number (except for Autonomous and Deregulated HEIs, State Colleges and Universities) and scanned picture and Remarks “For Board Examination Purposes” (First Timers and Repeaters) – Original and Photocopy
4. One (1) Certificate of Good Moral Character from any of the following:

5. Examination Fee: P600.00 – complete

Removal Examination shall be taken only three times (3x). If the applicant fails for the third time, he/she shall be required to take the entire examination.

Graduates of Bachelor of Science in Radiologic Technology may opt to take the Licensure Examination for X-ray Technology.

How to Apply for the Radiologic Technologist Board Exam

Step 1: Make Sure You’re Ready to Apply

Before you start applying, check if you meet these basic qualifications:

• ✅ You’re a Filipino citizen
• ✅ You hold a Bachelor of Science in Radiologic Technology
• ✅ You have all the required documents ready (see Step 2!)
• ✅ The application schedule for the RTLE is open ( visit the application schedule above!)

Step 2: Prepare These Documents and Requirements

Time to gather your documents! You’ll need both digital copies (for online upload) and printed copies (for PRC submission).

For All Applicants:

1. PSA Birth Certificate
2. Bachelor of Science in Radiologic Technology diploma or certificate
3. PSA Marriage Certificate (for married female applicants)
4. Transcript of Records (TOR) with your photo and the remark: “For Board Examination Purposes”
5. Valid NBI Clearance (for both first-time takers and repeaters)
6. Payment: ₱900.00 (complete)

Also make sure your payment method (GCash, Maya, etc.) has enough balance — the PRC online system charges a ₱20 service fee for digital payments.

Step 3: Create or Log In to Your PRC LERIS Account
